If you are undergoing a hardship or plan to relocate. St. Raymond's Cemetery offers our families the option to surrender their unoccupied Grave, Crypt or Niche back to the cemetery. The cemetery will refund you the money you have paid to date less a processing fee. This fee is currently $195.00 and is subject to increase annually. If you are interested in surrendering. Kindly complete the following form and steps below to expedite the process. 
You must be the original Purchaser (Deed Owner), spouse or direct bloodline heir to to initiate the surrender (some conditions may apply and will be discussed further by our staff upon reviewing your information below).

REQUIRED DOCUMENTATION

1. Notarized Letter of intent which states the deed owners intention. Required in the letter of intent: Full Name, address, phone number, Grave location, description of intent. Document must be notarized.

2. Check box - if the original Deed Owner (Purchaser) is still alive. Continue to step 3

2. Check Box - if the original Deed Owner (Purchaser) is deceased.

2. The following documents will be needed If the deed owner is deceased. • A copy of the Death Certificate if the deed owner is deceased. • The location of where the deed owner is interred. If cremated remains are not yet interred what are the plans?

2. If the original deed owner is deceased, the surviving descendants' intention is required on the letter of intent. The letter needs to be dated and has the address, phone number, location of grave and signatures of all individuals with burial rights to

• A copy of the requestor’s birth certificate as proof of the descendant of the deed owner. • All surviving children will need to sign an affidavit acknowledging that they are surrendering their burial rights. (death certificates of any deceased siblings)

3. Check Box - If you have a copy of your valid a driver’s license or passport.

3. Check Box - If you have a copy of the original contract and receipt if the account is not yet paid in full.

4. Mail the following to the address below. Notarized Letter of Intent, copy of valid photo ID, copy of the original deed, and copy of original contract .

4. Check Box if you are requesting a Lost Deed Affidavit. This needs to be completed, notarized and mailed with paperwork above.

OFFICE ONLY Check the files in the vault for any letters/affidavits for the location. e.g. assigned space, transfer of burial rights.

6. Grave/Crypt Surrender Affidavit (to be sent by the main office after the completion of Steps 1-3)

7. The Surrender Affidavit needs to be completed and notarized.

10. Original Deed, original notarized affidavits and supporting documents to be mailed to the main office.

11. A processing fee for the surrender will be deducted from the refund amount - currently $195.00

12. • A fee will be charged for the removal of a headstone if there is one on the plot. • For Crypts/Niches, a fee will be charged for the refacing of a slab if there is an inscription.

13. Main Office to review documents

14. Once steps 1,2 and 3 are completed and the documents are mailed and reviewed by the Main Office. We will then mail the applicable affidavits so that the rest of the steps can be completed.

Acceptable forms of payment include, Bank check, funeral home check, or all forms of credit cards. NO CASH or MONEY ORDERS
Should you arrive with Cash you may delay your appointment or be subject to rescheduling
Kindly contact your credit or debit card company to make sure you have sufficient funds prior to arrival
